Registering as a new patient requires a few simple steps:
To register with the practice fill out the Registration Form below. The information will be used to create a New Patient chart and we will verify your insurance.
After completing the Registration Form please have your primary care physician send the following:
An insurance referral (if required*)
Your last two office visit notes
Recent lab results
Any imaging related to the reason for your visit (if applicable)
Once everything is received, we will call you to schedule your appointment.
*For referrals, please use Dr. Costello’s NPI number: 1033144084. All documents should be faxed to 833-629-0822.
Note: We are a paperless office. Please do not send or bring paper records.
